Appendix C, Official Bid Form (August 2005) - as well as typing in the text this electronic document allows you to check the Cash Allowances boxes as necessary. Alternate Prices Attachment (optional form) - may be used as a supplement to Appendix C when there is insufficient space for all Alternate/Separate/Unit Prices. Faxed Bid Amendment (optional form) - may be used when submitting a Faxed Bid Amendment. Appendix E, Bonding Not Required Notice - used when Bids are less than $100, 000 and the Bonding Requirements of the Rules apply.
